The following forms have to be completed by all teams (please fax them to us as soon as you have completed them (250) 434-4595:

2. Liability Waiver for over the age of majority

3. Liability Waiver for under the age of majority

ALA Members (Teams from Alberta):

Any teams wishing to attend a National Competition must obtain approval from the Chair of the National and International Competition Committee before May 15 in box and Aug 7 for Field (except for Senior which is Sept 7) and prior to sending an application to the CLA Office.  No application will be approved where:

(a)     any player or other personnel on the roster is not in good standing;

(b)     the team has not complied with CLA Rules on coaching certification applicable to the Competition.
